ATHLETIC WAIVER & RELEASE OF LIABILITY

(ADULTS & MINORS – AMATEUR)

Karate Do Alliance League Florida / USANKF / WKF

I acknowledge that karate is a contact sport and that participation involves inherent risks of injury. I voluntarily assume all risks associated with my participation in this event.

I agree to comply with all rules, regulations, and decisions of the USA National Karate-do Federation (USANKF/WKF), the Karate Do Alliance League Florida, the organizing committee, and the officiating body.

I certify that I am in good physical condition and have no medical condition that would prevent my safe participation. For minors, this certification is provided by the parent or legal guardian.

I hereby release, waive, and hold harmless Karate Do Alliance League Florida, Sport Karate League, USANKF, event organizers, officials, referees, volunteers, sponsors, and venue representatives from any and all claims, liabilities, injuries, losses, or damages arising from participation in this event, including travel to and from the event.

I understand that failure to appear when my division is called, unsportsmanlike conduct, violation of competition rules, or possession/use of prohibited substances may result in disqualification without refund.

I grant permission for the use of my image, likeness, photographs, and video recordings taken during the event for promotional and informational purposes, without compensation.

By submitting this registration, I confirm that I have read, understood, and voluntarily agree to this waiver.

Reconozco que el karate es un deporte de contacto y que mi participación implica riesgos inherentes de lesión. Acepto voluntariamente todos los riesgos derivados de mi participación en este evento.

Me comprometo a cumplir con todos los reglamentos, normas y decisiones de la USA National Karate-do Federation (USANKF/WKF), la Karate Do Alliance League Florida, el Comité Organizador y el Cuerpo Arbitral.

Certifico que me encuentro en buenas condiciones físicas y de salud y que no existe impedimento médico que limite mi participación. En el caso de menores de edad, esta certificación es otorgada por el padre, madre o tutor legal.

Por medio del presente, libero y exonero de toda responsabilidad a la Karate Do Alliance League Florida, Sport Karate League, USANKF, organizadores, árbitros, oficiales, voluntarios, patrocinadores y representantes de la sede, por cualquier reclamo, lesión, daño o pérdida que pudiera ocurrir antes, durante o después del evento, incluyendo traslados hacia y desde el lugar del mismo.

Entiendo que la inasistencia al llamado de mi división, la conducta antideportiva, el incumplimiento del reglamento o la posesión/consumo de sustancias prohibidas podrán resultar en descalificación sin derecho a reembolso.

Autorizo el uso de mi imagen, fotografías y/o videos tomados durante el evento para fines promocionales e informativos, sin compensación económica.

Al completar este registro, declaro haber leído, comprendido y aceptado voluntariamente esta exoneración.
